Premium application forms

Create one form for staff, another for partnerships, and more for your teams. Each one has its own channels, reviewers, questions, and accept or reject actions.

PathDashboard → Your server → Premium Applications

Create a form​

  1. Click Create Form and enter a name up to 80 characters.
  2. Choose the publish channel and applications/log channel.
  3. Select at least one reviewer role and a cooldown from none to 7 days.
  4. Customize the message, Embed, and open-form button.
  5. Add between one and five short or paragraph questions.
  6. Configure DMs and roles to add or remove on acceptance and rejection.
  7. Save, then click Publish to Discord.

Customize the Embed title, description, color, image, thumbnail, and footer. The button can include text, an emoji, and a blue, gray, green, or red style.

Deletion is permanent

Deleting a form removes all its settings. Disable it instead when you only want to pause submissions.

Test the full workflow

Submit with a test account, accept it from the review channel, and verify the DM, role changes, and recorded decision.

This feature requires a Premium bot connected to the server.
